Description

Indications
Xenovate is indicated for use in the treatment of various dermatoses in adults, the elderly, and children over 1 year. It is effective in managing conditions such as:

• Psoriasis (excluding widespread plaque psoriasis)
• Recalcitrant dermatoses
• Lichen planus
• Discoid lupus erythematosus
• Other skin conditions that do not respond to less potent steroids.
  Use as prescribed by a registered healthcare professional.

Composition

• Clobetasol Propionate Cream: 0.05% (0.5 mg per gram)
• Clobetasol Propionate Ointment: 0.05% (0.5 mg per gram)
• Clobetasol Propionate Scalp Application: 0.05% (0.5 mg per gram)
• Clobetasol Propionate Shampoo: 0.05% (0.5 mg per gram)
• Clobetasol Propionate Lotion: 0.05% (0.5 mg per gram)
• Clobetasol Propionate Spray: 0.05% (0.5 mg per gram)

Pharmacology
Clobetasol Propionate is a highly potent topical corticosteroid with anti-inflammatory, antipruritic, and vasoconstrictive properties. It works by reducing allergic reactions, decreasing mast cell density, inhibiting eosinophil activation, and lowering cytokine production.

Dosage and Administration

• Cream/Ointment: Apply a thin layer to the affected skin areas twice daily. Use for short periods, with a maximum of 50 gm per week. Treatment should be limited to 2 consecutive weeks.
• Spray: Apply 1-2 times daily to the scalp, ensuring the total dose does not exceed 50 ml per week.
• Shampoo: Apply to dry scalp once daily, leave for 15 minutes before rinsing. Limit use to 4 weeks.
• Scalp Solution: Similar to the spray; avoid exceeding 50 ml per week.

Interactions
Drugs that inhibit CYP3A4 (e.g., ritonavir, itraconazole) may increase the systemic absorption of corticosteroids.

Contraindications

• Rosacea, acne vulgaris, and perioral dermatitis
• Primary cutaneous viral infections (e.g., herpes simplex, chickenpox)
• Hypersensitivity to the preparation
• Dermatoses in children under 1 year

Side Effects
Possible side effects include skin thinning, striae, and blood vessel changes with prolonged use. Some patients may experience pigmentation changes or hypertrichosis. If hypersensitivity occurs, discontinue use immediately.

Pregnancy & Lactation
The use of Xenovate during pregnancy should only be considered if the potential benefit outweighs the risks. It is unknown whether the drug is excreted in breast milk, so use with caution while breastfeeding.

Precautions & Warnings

• Prolonged use in children may lead to adrenal suppression.
• Avoid using on the face or in infants unless absolutely necessary.
• Do not apply near the eyes as it may cause cataracts or glaucoma.
