Description
Indications Xyflo is prescribed for the following conditions:
- Long-term prevention and management of asthma
- Acute prevention of Exercise-Induced Bronchoconstriction (EIB)
- Symptomatic relief of Allergic Rhinitis (AR), including both seasonal and perennial allergic rhinitis

Pharmacology Montelukast is a selective and orally active leukotriene receptor antagonist that blocks the cysteinyl leukotriene receptor (CysLT1). Cysteinyl leukotrienes (LTC4, LTD4, LTE4) are derivatives of arachidonic acid metabolism, released from cells such as mast cells and eosinophils. These leukotrienes contribute to asthma and allergic rhinitis symptoms by promoting airway inflammation, smooth muscle constriction, and tissue swelling. By inhibiting these receptors, Montelukast helps reduce airway inflammation and improve breathing.
Dosage & Administration
- Adults and adolescents (15 years and older) with asthma or seasonal allergic rhinitis: 10 mg once daily.
- Children with asthma or seasonal allergic rhinitis:
- 6 to 14 years: 5 mg once daily
- 2 to 5 years: 4 mg once daily
- 6 months to 5 years: 4 mg oral granules once daily (administered directly or mixed with cold water or soft food at room temperature)
Special Populations:
- Pediatric use: Montelukast is proven to be safe and effective in children aged 6 months to 14 years, with similar efficacy to that seen in adults.
- Hepatic insufficiency: No dosage adjustment is needed for mild-to-moderate hepatic impairment.
- Renal insufficiency: No dosage adjustment is necessary.
- Elderly patients: Montelukast’s pharmacokinetics and bioavailability remain unchanged in older adults, and no dose adjustment is required.

Drug Interactions Xyflo has been administered alongside other asthma and allergy treatments without an increase in adverse effects. Studies indicate that the recommended clinical dose does not significantly affect the pharmacokinetics of medications such as theophylline, prednisone, prednisolone, oral contraceptives, terfenadine, digoxin, or warfarin. However, enzyme inducers like phenobarbital may reduce Montelukast levels by approximately 40%. While dose adjustments are not generally required, monitoring may be needed when used with potent cytochrome P450 inducers such as phenobarbital or rifampin.
Contraindications
- Hypersensitivity to Montelukast or any of its components.
Side Effects
- Common: Diarrhea, fever, gastrointestinal discomfort, headache, nausea, vomiting, skin reactions, and upper respiratory tract infections.
- Uncommon: Anxiety, abnormal behavior, depression, dizziness, drowsiness, dry mouth, muscle pain, irritability, swelling, seizures, and sleep disorders.
- Rare: Angioedema, impaired concentration, disorientation, eosinophilic granulomatosis with polyangiitis, hallucinations, hepatic disorders, memory loss, palpitations, pulmonary eosinophilia, suicidal tendencies, and tremors.
Pregnancy & Lactation Montelukast crosses the placenta in animal studies, but there are no well-controlled studies in pregnant women. It should only be used during pregnancy if clearly necessary. As Montelukast may be excreted in breast milk, caution should be exercised when prescribing to nursing mothers.
Precautions & Warnings
- Xyflo is not intended for the treatment of acute asthma attacks or status asthmaticus; patients should have appropriate rescue medication available.
- Treatment with Xyflo can continue during asthma exacerbations, but it should not replace inhaled or oral corticosteroids without medical supervision.
- Xyflo should not be used as the sole treatment for exercise-induced bronchospasm.
- Patients with aspirin sensitivity should continue avoiding aspirin and nonsteroidal anti-inflammatory drugs (NSAIDs), as Xyflo does not prevent bronchoconstriction induced by these agents.
Overdose Effects Most reported overdose cases do not show serious effects. However, symptoms such as abdominal pain, drowsiness, thirst, headache, vomiting, and increased activity may occur. Supportive measures, including gastrointestinal decontamination and clinical monitoring, should be considered in overdose cases.
