Description

Indications: Xyflo 10 mg Tablet is indicated for the prophylaxis and chronic treatment of asthma, acute prevention of exercise-induced bronchoconstriction (EIB), and relief from symptoms of seasonal and perennial allergic rhinitis.

Pharmacology: Montelukast is a selective leukotriene receptor antagonist that blocks cysteinyl leukotrienes (LTC4, LTD4, LTE4), which play a key role in airway inflammation, edema, and smooth muscle constriction associated with asthma and allergic rhinitis. By inhibiting the CysLT1 receptor, Montelukast helps in reducing these inflammatory responses, leading to improved respiratory function.

Dosage & Administration: For adults and adolescents 15 years and older, the recommended dosage is 10 mg once daily. For children aged 6 to 14 years, the dosage is 5 mg once daily. For children 2 to 5 years, the dosage is 4 mg once daily. For infants aged 6 months to 5 years, 4 mg oral granules once daily can be taken directly or mixed with food or water. No dosage adjustment is required for patients with mild-to-moderate hepatic or renal insufficiency, and elderly patients do not require dose modifications.

Drug Interactions: Xyflo 10 mg Tablet has been safely used alongside other medications for asthma management without significant interactions. It does not alter the pharmacokinetics of commonly used drugs like theophylline, prednisone, oral contraceptives, NSAIDs, benzodiazepines, and decongestants. However, phenobarbital, a hepatic enzyme inducer, may reduce the effectiveness of Xyflo by approximately 40%, necessitating clinical monitoring.

Contraindications: This medication is contraindicated in individuals with known hypersensitivity to Montelukast or any of its components.

Side Effects: Common side effects include headache, nausea, vomiting, upper respiratory infections, diarrhea, skin reactions, and fever. Uncommon side effects may involve anxiety, dizziness, drowsiness, irritability, muscle pain, sleep disturbances, and dry mouth. Rare but serious adverse reactions include mood changes, hallucinations, suicidal tendencies, liver disorders, memory loss, palpitations, and eosinophilic granulomatosis with polyangiitis.

Pregnancy & Lactation: Montelukast crosses the placenta in animal studies, but adequate human studies are lacking. It should only be used during pregnancy if the benefits outweigh the risks. Since it is excreted in human milk, caution should be exercised when administering it to breastfeeding mothers.

Precautions & Warnings: Xyflo 10 mg Tablet is not a rescue medication and should not be used for the treatment of acute asthma attacks. Patients must always have a fast-acting bronchodilator available for emergencies. It should not be abruptly substituted for inhaled or oral corticosteroids, and dosage reduction of corticosteroids should be done under medical supervision. Aspirin-sensitive patients must continue avoiding NSAIDs, as Xyflo does not prevent bronchoconstriction caused by these drugs.

Overdose Effects: Overdose symptoms are usually mild and consistent with the drug’s safety profile, including abdominal pain, drowsiness, thirst, headache, vomiting, and hyperactivity. In case of overdose, standard supportive measures should be taken, such as removing unabsorbed material from the gastrointestinal tract and monitoring the patient.
