Description
Indications Xyflo is prescribed for:
- Long-term management and prevention of asthma symptoms.
- Prevention of Exercise-Induced Bronchoconstriction (EIB).
- Relief from symptoms of Seasonal and Perennial Allergic Rhinitis.

Pharmacology Montelukast is a selective leukotriene receptor antagonist that works by inhibiting the cysteinyl leukotriene receptor (CysLT1). Cysteinyl leukotrienes (LTC4, LTD4, LTE4) are derived from arachidonic acid metabolism and are released by cells such as mast cells and eosinophils. These leukotrienes contribute to airway inflammation, bronchoconstriction, and tissue swelling, leading to asthma and allergic rhinitis symptoms. By blocking these receptors, Montelukast helps reduce inflammation and improve breathing.
Dosage & Administration
- Adults and adolescents (15 years and older) with asthma or allergic rhinitis: 10 mg once daily.
- Children (6 to 14 years): 5 mg once daily.
- Children (2 to 5 years): 4 mg once daily.
- Infants and toddlers (6 months to 5 years): 4 mg oral granules once daily, administered directly or mixed with a spoonful of cold water or soft food.
Special Population Considerations:
- Pediatric Use: Proven safe and effective for children aged 6 months to 14 years.
- Hepatic Insufficiency: No dosage adjustment required for mild-to-moderate liver impairment.
- Renal Insufficiency: No dose modification necessary.
- Elderly Patients: No dosage adjustment needed, though the plasma half-life may be slightly prolonged.

Drug Interactions Xyflo has been safely used with various medications for asthma management. No significant interactions have been observed with drugs such as theophylline, prednisone, prednisolone, oral contraceptives, terfenadine, digoxin, and warfarin. However, caution is advised when used with phenobarbital or rifampin, as they may reduce Montelukast’s effectiveness by increasing metabolism.
Contraindications Xyflo should not be used in patients with known hypersensitivity to Montelukast or any of its ingredients.
Side Effects
- Common: Headache, nausea, vomiting, diarrhea, fever, skin reactions, upper respiratory infections, and stomach discomfort.
- Uncommon: Drowsiness, dizziness, irritability, anxiety, sleep disturbances, muscle pain, dry mouth, and abnormal behavior.
- Rare: Angioedema, hallucinations, memory loss, hepatic disorders, palpitations, pulmonary eosinophilia, and suicidal tendencies.
Pregnancy & Lactation Montelukast crosses the placenta in animal studies, but adequate human studies are lacking. It should only be used in pregnancy if necessary. Since many drugs pass into breast milk, caution should be exercised when prescribing Montelukast to breastfeeding mothers.
Precautions & Warnings
- Xyflo is not a rescue medication for acute asthma attacks and should not replace inhaled corticosteroids abruptly.
- Patients with aspirin sensitivity should continue avoiding aspirin and non-steroidal anti-inflammatory drugs (NSAIDs) while taking Xyflo.
- Regular monitoring is advised if used alongside hepatic enzyme-inducing drugs like phenobarbital.
Overdose Effects Most cases of overdose do not result in severe symptoms. Reported effects include abdominal pain, drowsiness, vomiting, headache, increased thirst, and hyperactivity. If overdose occurs, general supportive measures should be taken, such as gastric lavage and clinical monitoring.
