Indications:
Zemicef is used for treating bacterial infections, including:
- Upper Respiratory Tract Infections (URTI) such as otitis media and bronchitis.
- Urinary Tract Infections (UTIs) like cystitis and pyelonephritis.
- Conditions caused by common pathogens, including Streptococcus pneumoniae, E. coli, and Klebsiella species.
Pharmacology:
Cefixime, the active ingredient, is a third-generation cephalosporin. It works by inhibiting bacterial cell wall synthesis and is stable against beta-lactamase enzymes. Absorption is around 40-50% and unaffected by food.
Dosage & Administration:
- Adults & Children (10+ years or >50 kg): 200-400 mg/day, taken as a single or divided dose.
- Children (1-10 years): 8 mg/kg/day (doses vary by age).
- Typhoid in children: 5 mg/kg twice daily for 10-14 days.
- Renal impairment: Dose adjustments required if creatinine clearance <20 ml/min.
Contraindications:
- Known hypersensitivity to cephalosporins.
Side Effects:
- Common: Diarrhea, abdominal pain, nausea, vomiting, headache, dizziness.
- Rare: Pseudomembranous colitis, hypersensitivity reactions, and transient changes in liver/renal function tests.
Precautions:
- Use cautiously in penicillin-sensitive patients due to potential cross-allergy.
- Monitor for antibiotic-associated diarrhea.
- Avoid use in pregnancy and lactation unless essential.
Pregnancy & Lactation:
There are no adequate human studies, so use only if necessary. It’s unclear if Cefixime is excreted in breast milk.
