Description
Indications Zofen is prescribed for the preventive treatment of vascular headaches associated with migraines, including classical migraine, common migraine, and cluster headaches. However, it is not effective in alleviating migraine attacks once they have begun.
Description Zofen is a tricyclic compound that competitively inhibits serotonin receptors. It exhibits antihistamine, antibradykinin, and mild anticholinergic properties. Additionally, it has appetite-stimulating effects.
Pharmacology Pizotifen functions by inhibiting the peripheral effects of serotonin and histamine, which influence the permeability of cranial blood vessels and the release of plasmakinin. This mechanism alters pain thresholds associated with migraines. By blocking 5-HT receptors, pizotifen reduces serotonin-mediated cranial vasoconstriction and platelet aggregation. Additionally, it is believed to inhibit the peripheral actions of bradykinin. Pizotifen may also impede serotonin reuptake by blood platelets, leading to decreased passive distension of extracranial arteries. Its appetite-stimulating properties may be attributed to metabolic effects rather than direct stimulation of the appetite center.
Dosage & Administration
- Adults: The usual dose is 1.5 mg per day, taken as a single dose at bedtime or divided into three smaller doses throughout the day. Dosage may be adjusted based on individual patient response, up to a maximum of 4.5 mg per day. A single dose of up to 3 mg may be administered if required.
- Children (aged over 2 years): The recommended dose is up to 1.5 mg per day, usually divided into multiple doses. A single dose of up to 1 mg may be given at night if necessary.
Drug Interactions The sedative effects of Zofen may be enhanced when taken alongside sedatives, hypnotics, antihistamines (including some common cold medications), or alcohol. Additionally, Zofen may reduce the effectiveness of adrenergic neuron-blocking agents used for lowering blood pressure.
Contraindications Zofen should not be used by individuals with known hypersensitivity to pizotifen or any of its components.
Side Effects Common side effects associated with Zofen include increased appetite, weight gain, and drowsiness.
Pregnancy & Lactation There is limited information regarding the use of pizotifen during pregnancy, although no adverse effects have been reported. Therefore, it should only be used if the potential benefits outweigh any possible risks to the fetus. The safety of pizotifen during breastfeeding has not been established, so its use is not recommended for nursing mothers.

- Caution should be exercised in patients with urinary retention or renal impairment.
- Drowsiness caused by Zofen may impair the ability to drive vehicles or operate machinery.
