Description

Indications Zolium is prescribed for the treatment of the following conditions:

• Anxiety disorders
• Short-term relief of anxiety symptoms
• Anxiety associated with depression
• Panic disorder, with or without agoraphobia

Pharmacology Alprazolam belongs to the 1,4-benzodiazepine class and is a triazole derivative. It functions as an anxiolytic with sedative, hypnotic, and anticonvulsant properties. Alprazolam is believed to exert its effects through interaction with the Gamma-Aminobutyric Acid (GABA) receptor complex, enhancing GABAergic transmission. Like other benzodiazepines, it produces dose-dependent central nervous system (CNS) depression, ranging from mild sedation to hypnosis.

Dosage & Administration The recommended starting dose of Zolium is 0.25 mg to 0.5 mg taken three times daily. Based on patient response, the dose may be increased every 3 to 4 days in increments of no more than 1 mg per day. The maximum dose should not exceed 4 mg per day. In some cases, patients with panic disorder may require up to 10 mg per day, necessitating periodic assessment and possible dose adjustment.

Dosage should be tailored to achieve maximum therapeutic benefit with the lowest effective dose. If side effects occur at the initial dose, the dosage may be reduced. When discontinuing therapy, the dosage should be tapered gradually, decreasing by no more than 0.5 mg every three days.

For elderly patients or those with severe liver impairment, the initial recommended dose is 0.25 mg, taken two to three times daily, with gradual increases if necessary and well tolerated.

Patients transitioning from multiple daily doses of Alprazolam 0.25 mg or 0.5 mg to a single daily dose of Alprazolam 1 mg should take it once daily, preferably in the morning. Tablets should be swallowed whole and should not be chewed, crushed, or broken.

Drug Interactions The CNS depressant effects of Zolium may be intensified when used alongside other psychotropic drugs, anticonvulsants, antihistamines, alcohol, and oral contraceptives.

Side Effects Side effects, if they occur, are usually observed at the start of treatment and often subside with continued use. The most commonly reported side effects include:

• Drowsiness
• Light-headedness
• Depression
• Headache
• Confusion
• Dry mouth
• Constipation

Pregnancy & Lactation Alprazolam is classified as a pregnancy category D drug, indicating potential risk to the fetus. Therefore, its use should be avoided during pregnancy. As Alprazolam may be excreted in breast milk, breastfeeding is not recommended for mothers who require this medication.

Precautions & Warnings Zolium has the potential for psychological and physical dependence; therefore, dosage increases or abrupt discontinuation should only be done under medical supervision. The duration of treatment should be determined by the prescribing physician. Caution is advised when administering Zolium to patients with hepatic or renal impairment, chronic pulmonary insufficiency, or sleep apnea.

Use in Special Populations The safety and efficacy of Zolium in individuals under 18 years of age have not been established.

Overdose Effects Symptoms of Zolium overdose may include:

• Somnolence
• Confusion
• Impaired coordination
• Reduced reflexes
• Coma

In cases of overdose, general supportive measures should be implemented, including immediate gastric lavage.
