Indications
CoralCal-DX tablets are used in the treatment and prevention of various conditions, including:

  • Osteoporosis
  • Osteomalacia
  • Tetany
  • Hypoparathyroidism
  • Osteogenesis disorders

Additionally, CoralCal-DX tablets act as a supplement in cases where there is insufficient calcium intake in the diet, such as during childhood, rickets, pregnancy, lactation, or in elderly individuals. It is also indicated for managing pancreatitis and serving as a phosphate binder in chronic renal failure, among other conditions.

Composition
Each tablet contains:

  • Calcium Carbonate USP 1250 mg (from Coral source), equivalent to 500 mg of elemental Calcium
  • Vitamin D USP 200 IU

Each DX tablet contains:

  • Calcium Carbonate USP 1500 mg (from Coral source), equivalent to 600 mg of elemental Calcium
  • Vitamin D USP 400 IU

Each effervescent tablet contains:

  • Calcium Carbonate (from Coral source) USP 1500 mg, equivalent to 600 mg of elemental Calcium
  • Vitamin D3 (Cholecalciferol USP) 400 IU

Pharmacology
Calcium plays a crucial role in numerous bodily functions, and maintaining adequate levels in the blood is essential for health. Clinical research supports the use of calcium in preventing and treating osteoporosis and related fractures. Vitamin D is vital for healthy bones as it helps in the absorption of calcium from the gastrointestinal tract and stimulates bone formation. The combined effect of calcium and vitamin D is synergistic, aiding in bone growth and osteoporosis prevention. Coral-derived calcium in this preparation is structurally similar to human bone, ensuring better absorption compared to other sources. Vitamin D3 further enhances calcium absorption and helps in maintaining calcium balance in the body.

Dosage & Administration
The usual dose is one tablet once or twice daily, taken with plenty of water, or as directed by a healthcare provider. For optimal absorption, take the tablet after a meal. Dissolve one effervescent tablet in a glass of water, stir thoroughly, and drink the solution. It is advised to take the effervescent tablet with food for better absorption.

Interactions

  • Thiazide diuretics may reduce calcium excretion in urine, increasing the risk of hypercalcemia. Monitor serum calcium levels closely when using these medications together.
  • Systemic corticosteroids may reduce calcium absorption, and the calcium dose may need to be adjusted.
  • Medications such as cholestyramine (ion exchange resins) and paraffin oil (laxatives) may reduce vitamin D absorption from the gastrointestinal tract.
  • Calcium carbonate may interfere with the absorption of tetracycline antibiotics, so take these at least two hours before or four to six hours after calcium tablets.
  • Hypercalcemia may heighten the toxicity of cardiac glycosides, so monitor ECG and calcium levels.
  • If using bisphosphonates or sodium fluoride, take CoralCal-DX at least three hours before these medications.
  • Foods high in oxalic acid (e.g., spinach) or phytic acid (e.g., whole grains) may inhibit calcium absorption. Avoid taking calcium products within two hours of consuming these foods.

Contraindications
CoralCal-DX should not be used in the following conditions:

  • Hypercalcemia (high calcium levels)
  • Hyperparathyroidism
  • Hypercalciuria (high calcium in urine)
  • Nephrolithiasis (kidney stones)
  • Severe renal insufficiency
  • Hypersensitivity to any ingredient in the formulation
  • Concomitant use with digoxin requires careful monitoring of serum calcium levels

Side Effects
Calcium carbonate may cause mild gastrointestinal irritation and constipation. Rarely, hypercalcemia may occur, especially in patients with renal failure. Other potential side effects include allergic reactions, nausea, vomiting, irregular heart rhythms, decreased appetite, dry mouth, and drowsiness. During pregnancy, the daily intake of calcium should not exceed 1500 mg, and vitamin D should not exceed 600 IU (15 mcg). CoralCal-DX can be used during pregnancy and breastfeeding to address calcium and vitamin D deficiencies. Precautions & Warnings
Regular monitoring of serum calcium levels and renal function is recommended, especially for long-term users and those with underlying conditions. Elderly patients or those on concurrent treatments with cardiac glycosides or diuretics should be monitored closely. Discontinue if signs of hypercalcemia or impaired renal function appear. Use with caution in patients with sarcoidosis, as the condition may increase vitamin D metabolism. In immobilized osteoporosis patients, there is a risk of hypercalcemia. When prescribing additional calcium or vitamin D, consider the total intake to avoid overdose.

Overdose Effects
Overdosing on CoralCal-DX can lead to hypervitaminosis D and hypercalcemia. Symptoms of hypercalcemia include nausea, vomiting, constipation, abdominal pain, muscle weakness, fatigue, confusion, bone pain, kidney stones, and in severe cases, arrhythmias, coma, and death. Prolonged high calcium levels can cause irreversible renal damage and soft tissue calcification. Treatment involves stopping the medication, discontinuing other drugs that may contribute to hypercalcemia, and possibly using loop diuretics, bisphosphonates, calcitonin, or corticosteroids. Regular monitoring of electrolytes and renal function is essential.
