Indications
Diclofen is prescribed for the treatment of various conditions across multiple medical disciplines:

Rheumatology: Used in inflammatory and degenerative rheumatic conditions, including chronic involutive polyarthritis, ankylosing spondylarthritis, osteoarthritis, spondylarthroses, acute gout, and peri-articular rheumatic disorders.

Surgery and Traumatology: Effective for managing pain and inflammation related to sprains, bruises, dislocations, fractures, soft tissue injuries, and post-surgical interventions.

Obstetrics and Gynecology: Indicated for primary dysmenorrhea, episiotomy, adnexitis, endometritis, parametritis, salpingitis, and mastitis.

Otorhinolaryngology: Utilized as a pre-operative medication to prevent pain, inflammation, and swelling.

Dentistry: Prescribed for post-operative and post-traumatic pain, inflammation, and swelling.

Other Uses: Recommended for pain management and inflammation control in patients undergoing urogenital, renal, or biliary tract surgeries.

Pharmacology
Diclofenac Sodium is a potent non-steroidal anti-inflammatory drug (NSAID) with pronounced anti-rheumatic, anti-inflammatory, analgesic, and antipyretic properties. It also exhibits a mild uricosuric effect. Diclofenac functions by inhibiting prostaglandin biosynthesis, which plays a critical role in pain, inflammation, and fever. When taken with or after food, diclofenac is rapidly and completely absorbed from the gastrointestinal tract, reaching peak plasma concentrations within approximately two hours. At therapeutic doses, 99.7% of the drug binds to plasma proteins. Diclofenac undergoes hepatic metabolism and first-pass metabolism before systemic circulation.


Dosage & Administration

  • Diclofenac FC Tablet:
    • Adults: 75-150 mg daily, divided into 2-3 doses, preferably after meals. Long-term usage requires dose adjustment.
  • Diclofenac SR Tablet:
    • Adults: 1 tablet daily with liquid, preferably with meals. If needed, the daily dose can be increased to 150 mg with conventional tablets.
    • Children: 1-3 mg/kg body weight per day in divided doses.
    • Elderly: The lowest effective dose is recommended, despite no significant pharmacokinetic changes in elderly patients.
  • Diclofenac Dispersible Tablet:
    • Adults: The usual dose is 2-3 tablets per day, with a maximum daily dose of 150 mg. Milder cases may require only 2 tablets daily. Preferably taken before meals.
    • Children: Not generally recommended, except for juvenile rheumatoid arthritis (1-3 mg/kg daily). Tablets should be dissolved in half a glass of water before administration. No data is available for usage beyond three months.
  • Diclofenac TR Capsule:
    • Adults: One capsule daily, preferably taken after meals.
  • Diclofenac Suppository:
    • Adults: 50 mg suppository 2-3 times daily, with a maximum daily dose of 150 mg.
  • Diclofenac Injection:
    • Adults: Typically, 1 ampoule per day; in severe cases, up to 2 ampoules daily may be required.
  • Diclofenac Gel:
    • For External Use Only: Apply 2-4 g of gel to the affected area 3-4 times daily. Lightly rub the gel into the skin. It may be used in combination with other diclofenac dosage forms.

Drug Interactions

  • Lithium and Digoxin: Diclofenac may increase plasma concentrations of these drugs.
  • Anticoagulants: There are isolated reports of increased hemorrhage risk with diclofenac and anticoagulant therapy, though clinical studies show no significant effect.
  • Antidiabetic Agents: Can be co-administered with oral antidiabetic drugs without affecting their efficacy.
  • Cyclosporin: Cases of nephrotoxicity have been reported when used concomitantly.
  • Methotrexate: Co-administration within 24 hours may lead to serious toxicity.
  • Quinolone Antimicrobials: May increase the risk of convulsions when combined with NSAIDs. Caution is advised.
  • Other NSAIDs and Steroids: Concurrent use may heighten the risk of adverse effects.
  • Aspirin: Leads to decreased plasma levels of both drugs, though with unclear clinical significance.

Contraindications

  • Hypersensitivity to diclofenac or any of its components.
  • History of peptic ulcer disease.
  • Contraindicated in patients with asthma who experience exacerbation with aspirin or NSAIDs.
  • Diclofenac gel should not be used under occlusive dressings.

Side Effects
Diclofen is generally well tolerated, with mild and transient adverse effects. Possible reactions include:

  • Epigastric pain, nausea, diarrhea, or dizziness at the start of treatment.
  • Peripheral edema and mild skin reactions (rash, eczema).
  • Local irritation, redness, or rash when using Diclofenac Gel.

Pregnancy & Lactation
Diclofenac should be used during pregnancy only if absolutely necessary. The lowest effective dose should be administered. NSAIDs are not recommended during the first trimester. Due to limited clinical data, Diclofenac Gel is not advised during pregnancy. While trace amounts may be present in breast milk, no adverse effects on infants have been reported.


Precautions & Warnings

  • Patients with a history of gastrointestinal ulcers or bleeding should be closely monitored.
  • Elderly patients require careful supervision during treatment.
  • Diclofenac Gel should not come into contact with eyes or mucous membranes. Hands should be washed thoroughly after application.
