Indications
Siesta is prescribed for a variety of conditions, including:

  • Emotional disturbances such as acute anxiety, tension, and issues with interpersonal interactions. It is also effective for managing agitation, insomnia, and anxious-depressive reactions.

  • Cardiovascular and respiratory system disturbances, including pseudoangina pectoris, pericardial anxiety, tachycardia, emotional hypertension, dyspnea (difficulty breathing), and hyperventilation.

  • Gastrointestinal issues, including irritable bowel syndrome (IBS), epigastric pain, bloating, spasms, and diarrhea.

  • Urinary tract disorders, such as increased frequency, irritable bladder, and dysmenorrhea (painful menstruation).

  • Psychosomatic disorders, including psychogenic headaches, asthma, and gastric or duodenal ulcers.

  • Emotional reactions associated with chronic organic diseases.

Pharmacology
Siesta contains bromazepam, a potent psychotropic medication. At lower doses, bromazepam primarily alleviates tension and anxiety. At higher doses, it provides sedative and muscle-relaxant effects. Bromazepam works by binding to the GABA-A receptors in the brain, enhancing their inhibitory action, which results in a calming effect, without affecting other neurotransmitters.

Dosage
The typical starting dosage for outpatient treatment is 1.5 to 3 mg, taken up to three times a day. Initially, lower doses are recommended, gradually increasing to reach the optimal therapeutic level.

In more severe cases, especially in a hospital setting, the dosage may be increased to 6 to 12 mg, taken two or three times daily. Generally, treatment should not exceed 8–12 weeks, although extensions may be considered following a re-evaluation of the patient’s condition with expert advice.

For elderly or debilitated patients, particularly those with impaired liver function, lower doses should be considered.

For children, bromazepam is generally not recommended. If a healthcare provider deems it necessary, the dosage should be carefully adjusted based on the child’s body weight (approximately 0.1–0.3 mg per kg).

Administration
Siesta tablets are intended for oral administration. Follow the prescribed dosage and administration guidelines.

Interaction
When Siesta is taken alongside other centrally acting drugs, its sedative effects may be enhanced. Such drugs include antidepressants, hypnotics, narcotics, antipsychotics, sedatives, antiepileptic drugs, sedative antihistamines, and anesthetics. Additionally, combining Siesta with cimetidine may prolong the drug’s elimination half-life. It is highly recommended to avoid alcohol while taking Siesta, as it may intensify its sedative effects.

Contraindications
Siesta should not be used by individuals who are allergic to bromazepam or have the following conditions:

  • Severe respiratory insufficiency

  • Severe liver impairment

  • Sleep apnea syndrome

Side Effects
Common side effects associated with Siesta include:

  • Drowsiness

  • Fatigue

  • Muscle weakness

  • Numbness

  • Decreased alertness

  • Confusion

  • Headache

  • Ataxia (lack of coordination)

These side effects are generally more prominent at the beginning of treatment and tend to diminish with continued use. Anterograde amnesia (memory loss) may occur when taking therapeutic doses.

Pregnancy & Lactation
The safety of bromazepam during pregnancy has not been fully established. As bromazepam is excreted in breast milk, it should be avoided during lactation unless deemed absolutely necessary and prescribed by a healthcare provider.

Precautions & Warnings
The use of benzodiazepines, including bromazepam, can lead to physical and psychological dependence, especially when used for prolonged periods or at high doses. The risk of dependence is particularly high in patients with a history of alcohol use. Withdrawal symptoms, such as headache, muscle pain, anxiety, tension, confusion, and irritability, can occur when discontinuing the medication. To minimize these symptoms, it is advised to gradually decrease the dosage rather than stopping abruptly.

Siesta is not recommended for the primary treatment of insomnia caused by psychotic disorders. Additionally, caution should be exercised when engaging in activities that require alertness, such as driving or operating machinery.
